Vitrectomy is the surgical removal of the vitreous gel from the middle of the eye. Vitrectomy is a type of eye surgery that treats the disorders of the retina and the vitreous. Removal of the whole or part of the vitreous body in treating endophthalmitis, diabetic retinopathy, retinal detachment, intraocular foreign bodies, and some types of glaucoma.

Vitrectomy surgery often improves or stabilizes vision. The operation removes any blood or debris (from infection or inflammation) that may be blocking or blurring light as it focuses on the retina.

Your eye doctor will determine if the surgery can be done with local or general anesthesia. The surgery lasts 2 to 3 hours. Vitrectomy may require an overnight hospital stay, but it may sometimes be done as outpatient surgery.


How is it performed?

The initial step in this procedure is usually the removal of the vitreous gel through very small (1.4mm) incisions in the eye wall, hence the name "vitrectomy". The vitreous is removed with a miniature handheld cutting device and replaced with a special saline solution similar to the liquid being removed from the eye. After removing the vitreous gel, the surgeon may treat the retina with a laser (photocoagulation), cut or remove fibrous or scar stissue from the retina, flatten areas where the retina has become detached, or repair tears or holes in the retina or macula.

At the end of the surgery, silicone oil or a gas (perfluropropane) is injected into the eye to replace the vitreous gel and restore normal pressure in the eye.

Vitrectomy has been shown to greatly improve visual acuity in many people who have severe vitreous hemorrhage that has not cleared on its own. A vitrectomy can decrease the risk of severe bleeding in people who have begun to have bleeding into the vitreous gel.
